Job Description

We are seeking a dedicated and compassionate Social Worker to join our team and provide comprehensive social work services to youth and their families/caregivers. The ideal candidate will have strong clinical assessment skills, experience working with children and families, excellent documentation abilities, and a commitment to supporting positive outcomes for youth through individualized care planning, counseling, and case management services.

Compensation: Based on experience

The successful candidate will be organized, professional, and capable of managing multiple responsibilities while working collaboratively with a multidisciplinary team. He or she should possess strong communication skills, demonstrate sound clinical judgment, maintain accurate documentation, and respond effectively to changing situations and crisis situations.

Responsibilities

• Complete psychosocial assessments upon admission and monthly thereafter in accordance with established procedures

• Collaborate with multidisciplinary team members to develop individualized treatment and care plans within 30 days of admission and participate in ongoing evaluations every 90 days

• Gather information regarding youth support systems, prepare social histories, and submit documentation to supervisory staff

• Complete admission and discharge documentation, including environmental assessments and required regulatory paperwork

• Identify and address the emotional, behavioral, and social needs of youth throughout treatment

• Maintain and monitor financial eligibility information and assist families with eligibility-related concerns

• Participate in care planning meetings, patient status reviews, and interdisciplinary treatment team meetings

• Provide individual counseling, case management services, life skills training, and facilitate educational and therapeutic groups

• Coordinate services with community agencies, referral sources, schools, and other support systems

• Delegate social work-related responsibilities as appropriate

• Maintain timely and accurate clinical documentation and youth records

• Attend required trainings, staff meetings, and professional development activities

• Perform additional duties as assigned

Qualifications

• Master’s Degree in Social Work, Psychology, or a closely related behavioral health field (Required)

• Minimum of one year of child welfare, case management, or related experience preferred

• Strong written and verbal communication skills

• Ability to maintain confidentiality and adhere to all applicable laws, regulations, and ethical standards

• Excellent organizational and time-management skills

• Ability to work independently while contributing effectively as a team member

• Proficiency with documentation, electronic records, and computer systems

• Ability to manage crisis situations calmly and professionally

• Positive, cooperative, and professional attitude when working with youth, families, staff, and community partners

Physical Requirements

This position requires a combination of standing, walking, sitting, and computer work throughout the day. Responsibilities may include lifting up to 35 pounds, carrying materials, bending, kneeling, reaching, and occasional stair climbing. Candidates must possess the ability to communicate effectively with youth, families, staff, and community agencies, maintain accurate documentation, and perform duties requiring normal hearing, vision, and manual dexterity.
