What is Enhanced Care Management?

ECM is a Medi-Cal benefit through Partnership that gives a child or youth one Lead Care Manager who helps coordinate medical, mental health, school-related, and social-service needs in a “whole person” way.

Services are high-touch, community-based, and often in person where the child lives, learns, or feels comfortable (home, foster home, school, clinic, shelter, etc.)

WE WILL

  • Assess your needs, adjust plans, and provide hands-on help.

  • Provide emotional check-ins, practical logistics (e.g., rides to therapy), family education on behaviors or meds, and linking to resources, all tailored to your family’s goals

  • Involve you in Child and Family Team (CFT) meetings, coordinating with social workers in real-time, and ensuring seamless handoffs to prevent gaps in care.
  • Coach on de-escalating a youth’s trauma responses, model coping skills, and follow up with school staff the next day.
  • Will stay involved post-hospitalization through discharge, helping unpack meds, set up follow-ups, and connect to food or clothing supports until stability returns.

I want to continue with my current doctors or services?
ECM works with your current doctors, clinics, and all service providers to help you get coordinated care.

I’m unsure if my insurance is accepted?
To receive services, you must be covered by Partnership HealthPlan of California.

To enroll in Partnership HealthPlan of California, call your county Medi-Cal office or 1-800-300-1506. Your enrollment depends on your Medi-Cal type and county.

I have Medi-Cal but not Partnership?
If you have Medi-Cal and live in Sutter, Solano, or Yuba counties, you can choose or change your Medi-Cal health plan by calling Health Care Operations (HCO) at (800) 430-4263.

I’m unsure of eligibility?
If you work with a youth in foster care who has a lot of appointments, behavior challenges, or housing uncertainty, there’s a good chance they’re ECM-eligible.

Who is Eligible?

Children and youth involved in child welfare are a specific Population of Focus, including currently in foster care, recently in foster care within the last 12 months, youth in Family Maintenance, and youth who aged out of foster care up to age 26.

Many also qualify through other Populations of Focus, like serious mental health or substance use needs, risk of avoidable ER/hospital use, or homelessness/unstable housing.
