Children first is hiring a full-time Social Worker to provide individualized care to children and adolescents. Responsibilities may include: Performing comprehensive assessments, providing regular individual and group counseling, ensuring treatment plans are current, completing comprehensive progress notes, working collaboratively with those providing case management and/or psychiatric services.

Primary responsibilities:

• Complete psycho-social assessments of patients upon joining the facility and monthly in

accordance with established system

• Collaborate with multi-disciplinary team in devising care plan goals and approaches within 30

days of admittance to facility and evaluate every 90 days

• Gather information regarding patients’ support system, write a social history and submit to the

Social Worker Director

• Complete admission and discharge paperwork to include environmental assessments, submit to

required regulatory agencies and file in appropriate patient records

• Identify emotional and social needs of patients on a continuous basis

• Maintain financial eligibility information for each patient, counsel family/caregivers regarding

continued eligibility and update information as needed

• Attend all plans of care and patient status committee meetings as scheduled

• Delegate social work responsibilities to staff as needed

• Complete all scheduled in-service training sessions as required

• Employee provides individual counseling, case management, skills training, and facilitates

educational, skill development and counseling groups as assigned and properly and promptly

charts these activities in youth records

• Employee works cooperatively with other community resources and referral sources to coordinate

services to youth

• Other duties as assigned

The THP Social Worker provides social work case management to youth placed in the care of the CFFFA THP Program. This position requires a combined use of education gained through the earning of your degree as well as common sense.

Primary responsibilities:

•Weekly home visits

• One-on-one interaction with each client minimum twice per month

• Coordination / facilitation of medical, dental, mental health, school, and other

appointments/services necessary for client’s success

• Development of initial ANSP (Appraisal, Needs, & Services Plan) and quarterly updates thereafter

• Development of on-site real-time case note at home/face-to-face visits

• Thorough and timely SIR (Special Incident Report)

• Professional communication with service providers

• Attendance and participation at meetings as necessary

• Attendance and participation at group supervision

• Seeking out guidance and direction from supervisors/leaders/management as needed

• Accept coaching from co-workers, supervisors, management, and Administrator

• Engage in timely and accurate communication with service providers involved in client’s case

• Initiate communication with supervisors as you receive information impacting a case

• After-hours on-call rotation

• Adhere to Mandated Reporter laws

• Coordinate and participate as necessary in the delivery and/or completion of assessments

• Coordinate and assist with a vast array of crisis intervention plans and techniques

This position provides direct service to children between the ages of 3 to 19 years of age whose behaviors, placement history, and situations have kept them from achieving successful outcomes.

Primary responsibilities:

• Travel to foster child/ RFA home

• Participation in staff meetings and other related meetings as necessary

• Work with Treatment Team, agency Social Workers, and other designated service providers

• Attend Treatment Team meetings as necessary per Social Work Supervisor

• Consult with agency Social Worker, Social Work Supervisor, and Agency Director as needed

• Effectively communicate information

• Demonstrate self as a positive role model

• Respond calmly and professionally to crises

• Respond to crisis situations on short notice

• Have intervention/ crisis management toolbox readily available

• Respond to crisis situations on evenings and weekends

• Report to agency Social Worker / Social Work Supervisor behaviors and concerns

• Contribute to and follow a plan for partnering with a foster family

• Maintain adult/child boundaries

• Consult with FFA SW, SW Supervisor, or Agency Director to call the police if de- escalation techniques do not work or if the client is a danger to self or others

• Encourage child involvement in positive activities, i.e., hobbies, music, sports, community groups

• Use supervision and feedback to enhance job skills and grow professionally

• Proficiently take notes and transfer into written documentation

• Adherence to emergency on-call protocol (general, runaway, 5150, emergency room visits, disaster)

• Take responsibility for implementing interventions

• Take responsibility for modifying interventions to fit child and situation

• Research and implement new interventions on a regular basis

• Provide safe, effective, efficient culturally competent support services

• Provide crisis response when approved by SW Supervisor or Agency Director

• Transport child to appointments or to designated location as needed

• Protect the “Personal Rights” and dignity of the child

• Develop a strong understanding of the ISFC program

• Have a clear understanding of the WIC requirements

• Complete entire ISFC training program within two months of starting position

• Obtain Social Work Supervisor approval for “odds-n-ends” type tasks – i.e., transporting

• Take responsibility for overseeing the check-out and check-in of training modules for on-

going ISFC foster homes as well as new homes seeking ISFC certification

• Support Specialist services may be used for general foster care cases on a limited and Supervisor or Director pre-approval basis

Job Description

Children First Counseling Center and Lassen Counseling Services has an immediate need for LMFT’s or LCSW’s to work out of our Red Bluff. Therapists must have the experience to take on a caseload quickly and he/she should understand/be proficient in caseload documentation for a variety of funding sources.

Compensation: Based on experience

The ideal candidate would have self-confidence, good organizational, computer, and writing skills, a professional appearance, and be able to manage their own caseload scheduling. He/She would be able to take direction, ask questions, and participate as a team member.

-Supervision by an experienced Clinical Supervisor
-Ability to accrue hours required to apply for licensure (if not already licensed)
-Expand knowledge base/clinical training of evidence-based practices
-Develop assessment and documentation skills
-Work with a variety of clients including school-based, groups, adults, etc. (depending on existing experience and interest)

Requirements:

-Masters Degree in a field of behavioral science from an accredited educational institution

Children First Counseling Center is the primary counseling center serving all of rural Tehama County and the incorporated areas of Los Molinos, Corning, and Red Bluff where our main offices are located. Services are provided to individuals of all ages, plus families, couples, and groups through a variety of contracted funding sources, including Medi-Cal, Managed Care, private insurance, Victim Witness, and CAPIT (CPS).

Lassen Counseling Services, a subsidiary of Children First Foster Family Agency, is a non-profit agency that provides counseling services to a variety of clients including: children, adolescents, adults, and seniors. We serve Medi-Cal Managed Care (Partnership/CA Health & Wellness), private insurance, and school-based clients in Shasta, Tehama, Siskiyou, and Lassen/Modoc counties.
